1. Terms are subject to change without notice and will be made available upon request.

💳 Payments

3. Upon registration, the parent or legal guardian will decide upon a Monthly, Biweekly, or Weekly payment plan.

4. Payments are due on the Friday before the week of service and considered late after the Wednesday of that week.

5. If late payment results in legal action, the parent/guardian listed will be liable for all resulting charges.

6. Tuition can be paid via: Zelle, Cash App, Venmo, PayPal, Cash, Check, Debit/Credit Card.

7. Set up AutoPay to avoid late fees — complete the payment page.

8. The Refer a Friend program allows a student to bring a friend or family member for a trial class.

9. No refunds will be given; your account will receive a credit.

🎨 Classes

10. Students will not be allowed to attend classes of a higher age group or level unless explicit permission is given by a senior instructor.

11. Students may attend more than one class per day with an additional fee.

🏆 Evaluations & Advancement

15. Evaluations run every 4–12 weeks and are held during after-school hours or on weekends.

16. Payments for evaluations are due prior to or on the day of the event.

🚌 School Pickup

18. The bus driver maintains a current roster. The bus leaves 5–15 minutes after each school's release time.

19. Parents must notify staff of any schedule changes. A special pickup fee applies for missed buses.

🏥 Emergencies & Health

20. In emergencies, we will contact parents and secure treatment by calling 911. Alternate arrangements require documentation.

21. I confirm my child is in good health and I will notify the program of any infectious disease exposure.

📸 Photo Release

22. I give permission for The Arts & Star Center LLC to use photographs of my child for promotional material.

In exchange for participation in programs offered by THE ARTS & STAR CENTER LLC, of 6142-6144 North US 41, Apollo Beach, FL 33572, and/or use of the property, facilities, and services, I agree for myself (if applicable) and for the members of my family, to the following:

1. I agree to observe and obey all posted rules and warnings, and further agree to follow any oral instructions or directions given by The Arts & Star Center LLC staff.

2. I recognize that there are certain inherent risks associated with participation in these programs. I assume full responsibility for injury and further release and discharge The Arts & Star Center LLC for injury, loss, or damage arising out of my or my family's use of or presence upon the facilities, whether caused by the fault of myself, my family, or the staff.

3. I agree to indemnify and defend The Arts & Star Center LLC against all claims, causes of action, damages, judgments, costs, or expenses, including attorney fees and other litigation costs which may in any way arise from my or my family's use of or presence upon the facilities.

4. I agree to pay for all damages caused by my or my family's negligent, reckless, or willful actions.

5. Any legal or equitable claim that may arise shall be resolved under Florida law.

I have read this document and understand it. I further understand that by signing this release, I voluntarily surrender certain legal rights as listed above.
